当クリニックでは、お母さまのご希望を伺いながら、赤ちゃんの安全を第一に考え、分娩の進み具合やお母さまの状態を確認したうえで対応しております。
無痛( 和痛 )分娩とは
無痛( 和痛 )分娩とは、硬膜外麻酔という方法を用いて、陣痛の痛みをやわらげる分娩方法です。
背中から細い管を入れ、そこから麻酔薬を少しずつ投与することで、出産時の痛みを軽減します。
痛みの感じ方には個人差がございます。完全に痛みがなくなるというよりも、痛みをやわらげ、分娩時の負担を軽くすることを目的としております。
硬膜外麻酔を使用する理由
無痛( 和痛 )分娩では、一般的に硬膜外麻酔が用いられます。
硬膜外麻酔は、お母さまの痛みをやわらげながら、赤ちゃんへの影響をできるだけ抑えられる方法です。
全身麻酔や点滴による麻酔では、赤ちゃんにも麻酔の影響が及び、出生後の呼吸に影響する可能性がございます。そのため、分娩時の痛みをやわらげる方法として、硬膜外麻酔が選択されます。

無痛( 和痛 )分娩には、以下のようなメリットがございます。
- 陣痛の痛みをやわらげることができる
- 陣痛への恐怖心や不安を軽減しやすい

無痛( 和痛 )分娩を受けるための確認事項
無痛( 和痛 )分娩を安全に行うためには、事前にお母さまのお身体の状態を確認する必要がございます。
硬膜外麻酔では、背中から細い管を入れるため、血液が固まりにくい状態では実施できない場合がございます。そのため、事前に血液検査を行い、血小板数や血液の固まりやすさなどを確認します。
また、以下のような場合は、硬膜外麻酔が難しいことがございます。
- 脳や脊髄に病気や炎症がある方
- 全身に感染の兆候がある方
- 背中や腰の穿刺部付近に炎症がある方
- 出血しやすい体質の方
- 背骨に変形や異常がある方
- 血液を固まりにくくするお薬を内服している方
無痛( 和痛 )分娩をご希望の場合も、すべての方に実施できるわけではありません。診察や検査結果をもとに、医師が安全に実施できるかを確認いたします。
無痛( 和痛 )分娩のリスクについて
無痛( 和痛 )分娩には多くのメリットがございますが、医療行為である以上、リスクがまったくないわけではありません。
硬膜外麻酔に伴う主な症状や合併症として、以下のようなものがございます。
- 血圧低下
- 発熱
- かゆみ
- 頭痛
- 神経症状
- 局所麻酔薬中毒
- 麻酔が強く効きすぎること
- 硬膜外血腫・感染
- アレルギー反応

また、無痛( 和痛 )分娩中は痛みがやわらぐ一方で、分娩の進行状況や赤ちゃんの状態を慎重に確認する必要がございます。
分娩の進行がゆっくりになる場合や、胎児心拍の変化、発熱、微弱陣痛、分娩停止、出血などに注意が必要です。

また、必要に応じて、分娩誘発や陣痛促進についてもご説明し、内容をご理解いただいたうえで、文書による同意をいただいております。

当クリニックでは、原則として計画分娩による分娩誘発・陣痛促進を行いながら、無痛( 和痛 )分娩に対応しています。
ただし、自然に陣痛が始まった場合や、破水で入院された場合にも、可能な限り対応いたします。
一方で、院内の分娩状況や急を要する患者様の対応などにより、状況によっては無痛( 和痛 )分娩に対応できない場合がございます。あらかじめご了承ください。
無痛( 和痛 )分娩の費用について
無痛( 和痛 )分娩は保険診療ではなく、自費診療となります。
出産に至るまでの時間や分娩時刻など、状況により費用は異なりますが、目安として通常の分娩費用に加えて、約10万円の追加費用がかかります。

At Iwasawa Clinic, we offer painless and pain-reduced delivery using epidural anesthesia.
Painless delivery is a method intended to reduce pain during childbirth, helping to ease physical exhaustion and emotional stress so that mothers can approach delivery as calmly as possible.
At our clinic, we listen to each mother’s wishes while placing the safety of the baby first. We provide care after confirming the progress of labor and the mother’s condition.
What is painless or pain-reduced delivery?
Painless and pain-reduced delivery is a childbirth method that uses epidural anesthesia to ease labor pain.
A thin catheter is inserted through the back, and anesthetic medication is administered little by little through the catheter to reduce pain during childbirth.
The way pain is felt varies from person to person. Rather than completely eliminating pain, the purpose is to ease pain and reduce the physical burden during delivery.
Why epidural anesthesia is used
Epidural anesthesia is commonly used for painless delivery.
This method can reduce the mother’s pain while minimizing the effect on the baby as much as possible.
With general anesthesia or intravenous anesthesia, the anesthetic may also affect the baby and could potentially influence the baby’s breathing after birth. For this reason, epidural anesthesia is selected as a method to ease labor pain.
Benefits of painless delivery
Painless delivery may offer the following benefits:
- Labor pain can be reduced
- Fear and anxiety about labor may be eased
- Physical exhaustion during delivery may be reduced
- Postpartum fatigue may be reduced in some cases
- Depending on the situation, it may be easier to transition to an emergency cesarean section
It can be one option for those who are especially anxious about pain or wish to preserve as much physical strength as possible during childbirth.
Points to confirm before receiving painless delivery
To perform painless delivery safely, it is necessary to check the mother’s physical condition in advance.
Because epidural anesthesia involves inserting a thin catheter through the back, it may not be possible if the blood does not clot easily. For this reason, blood tests are performed in advance to check platelet count and how easily the blood clots.
Epidural anesthesia may be difficult in the following cases:
- Those with disease or inflammation of the brain or spinal cord
- Those with signs of systemic infection
- Those with inflammation near the puncture site on the back or lower back
- Those who tend to bleed easily
- Those with spinal deformities or abnormalities
- Those taking medication that makes the blood less likely to clot
Even if you wish to have painless delivery, it may not be possible for everyone. Based on examination and test results, the doctor will determine whether it can be performed safely.
Risks of painless delivery
Painless delivery has many benefits, but as with any medical procedure, it is not completely without risk.
Main symptoms and complications associated with epidural anesthesia may include:
- Low blood pressure
- Fever
- Itching
- Headache
- Neurological symptoms
- Local anesthetic toxicity
- Excessive effect of anesthesia
- Epidural hematoma or infection
- Allergic reactions, etc.
During painless delivery, while pain is reduced, it is also necessary to carefully monitor the progress of labor and the baby’s condition.
Attention must be paid to delayed labor progress, changes in the fetal heart rate, fever, weak contractions, arrest of labor, bleeding, and other concerns.
For this reason, an appropriate observation system is important during painless delivery so that complications or abnormalities can be detected as early as possible.
Our safety management system
At our clinic, our staff acquire the necessary knowledge and skills to perform painless delivery safely, and we conduct ongoing training and practice.
Staff involved in the management of painless delivery have completed the J-CMELS Basic Course and JALA Category A training.
We also work to improve our in-house knowledge and skills so that we can recognize and respond appropriately to complications that may be associated with painless delivery, such as local anesthetic toxicity, excessive anesthesia effect, uterine atony bleeding, and placental abruption.
Advance explanation and consent
For those who wish to have painless delivery, we explain painless delivery using epidural anesthesia during prenatal checkups and painless delivery classes.
When necessary, we also explain labor induction and labor augmentation. After confirming that you understand the details, we obtain written consent.

Painless delivery at our clinic
As a general rule, our clinic provides painless delivery together with labor induction and augmentation through planned delivery.
However, if labor begins naturally or you are admitted after your water breaks, we will respond as much as possible.
Please note that depending on the delivery situation in the clinic or the need to respond to urgent patients, we may not be able to provide painless delivery in some cases.
Cost of painless delivery
Painless delivery is not covered by health insurance and is provided as self-pay medical care.
The cost may vary depending on the time required until delivery, the time of delivery, and other circumstances. As a guideline, an additional fee of approximately 100,000 yen is required in addition to the usual delivery cost.
